Do I need a referral to see your practitioners?

You do not need a medical referral in order for our team to assess or treat you. However, please ensure or clarify whether your insurance may or may not require a medical referral to reimburse you for the treatments.

What documentation should I bring to my appointment?

For your appointment please bring necessary documentation such as your health card or a piece of ID. You can bring your medical reference and/or all relevant documents (x-ray results, magnetic resonance, scan, etc.).

For people who have had an accident at work (CNESST) or on the road (SAAQ), we must first have the authorization of the paying agency. We recommend that you send us your work declaration or any documents issued by the paying agency or the doctor in charge.

What should I expect in my first physiotherapy appointment?

We suggest that you arrive 5 to 10 minutes before your assessment in order to complete our health questionnaire and provide us with all the documents relevant to the reason for your consultation.

Subsequently, the physiotherapist will question you about your overall health and the reason for your consultation, and your goals.

Our physiotherapists will then make an evaluation of your condition by observations, analyzes of your biomechanics and your strength, and by palpation of the structures affected.

The therapist will then be able to give you and explain to you his clinical impression of your condition. A treatment plan will be explained and decided with you.

How should I dress for my appointments?

Loose, comfortable clothing is recommended to properly expose the area to be assessed i.e. for a leg injury bring shorts, for a shoulder injury you can bring a tank top.

How many visits will be necessary?

The number of visits varies from patient to patient, and especially according to your goals. Our goal is to provide you with a treatment plan that is efficient for your recovery so that you are able to return to full health as soon as possible. We will be able to give you an overall idea at the point of assessment.

Does the insurance cover the treatments?

Most insurance may cover physiotherapy treatments. For massage therapy, osteopathy, and nutrition appointments, your insurance can also reimburse everything under the naturopathic heading. Check with your insurance provider regarding your coverage.

How can I see a Sports Medicine Doctor at the Polyclinic 819

To have a consultation with one of our sports medicine doctors, your doctor must send a consultation or referral request by fax to the Polyclinic 819.
